Shortly after admission to the care facility the Interdisciplinary Care Team will meet with you and your family to develop a Care Plan, including nursing, therapy, nutrition, activities, social series medical data to likes and dislikes, personal history and interests. Once a plan is in place frequent updates are given to maintain constant communication and ease any concerns.
The therapy staff is part of the “bricks and mortar” of Brookside Healthcare Center, and the therapists take pride in the performance of the therapy department as well as the entire facility. They understand each patient has unique and special needs, so the in-house therapy program is completely customizable to meet those needs. Wellness is a collaborative effort among the team members, who work with your physician to create a unique vision and treatment plan for you. They can even provide specialized equipment to aid in your treatment.
They have a full-time in-house therapy team – not outside contractors or agencies – so patients get to know their therapists, and vice-versa. Having the same therapy team working with you every day is a big plus, and they always seek to achieve just the right “match” between patient and therapist, for optimal results and comfort.
The therapy team is passionate about bringing the latest techniques and programs to their patients. Through continuing education, the physical and occupational therapists apply the latest research to improve balance and reduce fall-risk. They also utilize therapeutic modalities combined with a comprehensive therapy program for strengthening, balance training, pain reduction, wound-healing, urinary incontinence, and increasing range of motion.
In addition, they have therapists certified in specialty areas of practice such as: • Wound Care • Neurological Conditions • Electrical Stimulation For Swallowing Dysfunction • Lymphedema • Pulmonary Programs • Speech-language Therapy
